Since 1985, the Lifeline program has provided a discount on phone service for qualifying low-income consumers to ensure that all Americans have the opportunities and security that phone service brings.

Please complete this form and return to RCN. Requests may take up to 14 business days in order to be processed. The request will be denied if the required documentation does not accompany this form. Submitting false documentation or misrepresenting your identity may result in legal action in which case you may be penalized to the fullest extent of the law.


  • Lifeline applies to telephone service, only.
  • The name on the bill must match the name of the household member participating in the Lifeline program.
  • Lifeline must be renewed yearly; customers receive a reminder letter from RCN 30 days prior to the yearly renewal date.
  • Customers must follow the same criteria for filling out the Lifeline application each year.
  • Qualified candidates can only maintain one telephone line with a lifeline discount.
